APP for Flex/PRN Palliative Care at SCH/SWRMC - Virginia

Sentara Medical Group is seeking a Flex/PRN Nurse Practitioner or Physician Assistant to join its Palliative Care teams at Sentara Careplex Hospital and Sentara Williamsburg Regional Medical Center in Virginia . The APP is responsible for providing clinical care, support, and education for the palliative patient population as part of the health care team.
Position Highlights:

• PRN/Flex
• Scheduling by mutual agreement, 8 hour shifts between the hours of 7a - 5p
• Performing palliative care consultations and follow up visits for hospitalized patients with acute palliative care needs including symptom management and complex medical decision making
• Facilitating family meetings related to current medical decision making and advance care planning
• Assisting with developing appropriate discharge plans with respect to established goals of care including close collaboration with Hospice for patients to ensure access and continuity of care.
