MENTAL HEALTH SPECIALIST I - CSS ENCC Exceptional Need Care Coordinator
Mental Health Program (Classification 771)
SEIU Represented Full Time (37.5 hours/week) position
Job Summary
  • This position supports residential care coordination efforts within the larger mental health system. Working with multiple agencies and systems to support clients in therapeutic, least restrictive environments by facilitating community integration. This position is a combination of administrative and clinical duties.
  • Participate in all CHOICE model meetings and screenings.
  • Complete CHOICE model referrals (PRTS application, LOCUS/LSI, and clinical documentation)
  • Provide services and supports, such as individual and group therapy, that lessen or eliminate the debilitating symptoms of mental illness and minimize or prevent acute episodes of the illness
  • Promote recovery principles, dignity, hope, and well-being
  • Foster community integration and encourage use of natural supports
  • Help individuals to establish economic self-sufficiency and housing stability by facilitating residential care coordination meetings and participate in residential monthly staff.
  • Provide co-occurring mental illness and substance abuse treatment
  • Provide ongoing assessment of consumers' substance use status, ADL/IADL residential care needs and provide stage-wise individual and group interventions both in the office and in the community
  • Conduct comprehensive assessments annually and update as needed
  • Assume primary responsibility for developing, writing, implementing, and revising service plans annually and update as consumer's recovery needs change.
  • Collaborate with Community Support Services, Assertive Community Treatment, and Precommitment Investigator teams to determine appropriate level of care individua needs in the community.
  • Other job duties related to Community Support Services Treatment Team may include working with Community Support Services Team, Psychiatric Security Review Board, Rental Assistance Services and other related program services

Minimum Qualifications
Must be able to pass a criminal history check and possess a valid Oregon drivers' license. Bilingual Spanish skills a plus. Requires successful completion of a Master's Degree in psychology, social work, and counseling or in a related field.
