As a CASE MANAGER, you will maintain a high standard of quality client care, by ensuring that the program is being utilized most appropriately for each client.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Include the following; however other duties may be assigned:

  • Responsible for having a clear and thorough knowledge of the state and community based programs
  • Manage caseload in the program to include home visits, telephone contact and written communication with clients on a monthly basis.
  • Maintain client records per policies, regulatory, etc requirements
  • Perform chart reviews as required
  • Attend meetings with collaborative agencies.
  • Research and maintain a resource list of non-waiver community resources to better serve clients.
  • Maintain client confidentiality.

Required Skills

  • Must have formal education equivalent of a bachelor’s degree in health and human services field and two years work experience in delivery of services and supports to persons with related conditions or similar disabilities or
  • A high school diploma and four years work experience in the delivery of services and supports to persons with related conditions or similar disabilities; or
  • A high school equivalency certificate issued in accordance with the law of the issuing state and four years work experience in the delivery of services and supports to persons with related conditions or similar disabilities;
  • is an employee of the CMA; is not employed by or contracting with a DSA to provide a direct service to an individual served by the CMA and is not a relative of the individual to whom the case manager is providing case management.
  • Oral and written communication skills and the ability to work independently.
  • Bilingual skills preferred (Spanish).
